Senior Transcript Release Form Authorization - This form needs to be signed by BOTH the Hoffman student and his/her parent/guardian (if student is under 18 years old) before Hoffman will release any transcripts - (effective beginning with the class of 2007)

    Transcript Request Form - This form should be used for a paper college application or a scholarship request

    Emergency Transcript Form - This form should be used if a transcript needs to be sent before or without meeting with a counselor
